By submitting a photo image through the Colorado Mesa University MAVcard Photo Upload web site, you are agreeing to these terms and conditions. If you do not accept these terms, please do not proceed with the image submission.
If you choose not to proceed, you will be required to visit the MAVcard office in the University Center to have your photo taken and your MAVcard produced. We reserve the right to update these terms and any changes will be effective immediately.

About MAVcard: The MAVcard is the official ID card of the University and is managed by the MAVcard Office under the University Center, a department of Student Services. The MAVcard is the property of Colorado Mesa University and its use is governed by the University's regulations and policies.

How we will use your photo image:
The digital photo image that you submit via this website will be used to produce your MAVcard.
The Colorado Mesa University MAVcard is the offical student ID at Colorado Mesa University.
The MAVcard has many purposes and allows students, faculty, and staff access to the:
- Library
- Recreation Center
- Campus Dining Outlets with meal plans
- Patment convenience at several campus departments using FLEX money
- CMU Athletic events
- Pre-approved after hours building access
- Residence Hall entrance

Other uses of your image: By submitting this photo for your student ID you agree this photo may be used by Colorado Mesa University Student Records and or the University Health Service.

MAVcard Policies:
- The card holder is responsible for the cost of lost and replacement cards.
- Do not lend your card to anyone for any purpose.
- Lost cards must be reported to MAVcard office immediatly. Go to http://www.coloradomesa.edu/mavcard/lost-card.html for more information about lost or stolen MAVcard.
- MAVcards are property of Colorado Mesa University.
